Email Communications Forms - All Choirs

Students and parents of all HMS choir students must complete the email communications form in order to receive email communications about HMS choir and Patriot Singers.  Each choir has their own form.  Complete each form for which your student is a member for each email you wish to receive communications to.  Click on the link and follow the directions for your student's choir.

Email Communications & Emergency Contact

Patriot Singer Parents & Students:

Complete the following 2 forms.  These forms are separate and MUST be completed ASAP.

Email Communications 
This will be used to send communications on a regular basis as well as other information needed throughout the school year.  You will need to complete the form for each one of the email addresses to which you wish to receive information about HMS Show Choir.

Emergency Contact Information
This will give Mr. Litten and school administration access to mobile emergency contact information that can be used in the event of an emergency.  Be sure to complete the entire form.  If your contact information changes throughout the school year make sure to email with the changes.

I'll see you on Tuesday, August 16 @ 5:15pm for a brief informational meeting in the HMS Choir Room.

2016-2017 Patriot Singers Email Communications

Members and parents/guardians of the 2016-2017 Patriot Singers need to sign up for email communications.  Parents AND students are REQUIRED to sign up for email communications from the Patriot Singers.  This is the main way for information sharing for the show choir.

Other family and friends are welcome to sign up for email information.
